Parents/guardians: please fill out all of the following data and click the "Submit" button when you are finished. Each parent is welcome to fill out a separate questionnaire, if desired.
The goal of this assignment is to help me get a sense of your expectations for your child in chemistry class. This information will be invaluable in helping me to meet your needs as parents, as well as those of your child.
This questionnaire should take you about 5 minutes. Please answer the questions honestly; I do not believe that there are any "good" or "bad" answers. For example, if you rarely check in with your child about homework, it might be because you have never needed to do so.
Finally, please note that even though there are no "right" answers to these questions, the completed questionnaire counts as a homework assignment, and will be counted as part of your child's chemistry grade.
